Welcome to the Knappa School District HRA for the 2025-26 Plan Year!

 

Knappa School District pairs Moda Medical Plan 5 with an employer and union coordinated Group Health Reimbursement Arrangement (HRA) to provide a lower deductible and lower maximum out of pocket. Through the HRA claims process, you and your covered eligible dependents may be able to receive reimbursements for healthcare expenses!

Filing Claims

Great news! An automatic claims feed is in place between Moda Health and Diversified Benefit Services (DBS) for Moda Medical Plan 5 Group HRA participants in your district, so there is no need to file Group HRA claims for dates of service in the 10/1/2025 – 9/30/2026 Plan Year unless a covered, eligible member of your family has double coverage for medical expenses. If you have questions about your claims for dates of service in the October 1, 2025 – September 30, 2026 Plan Year, please be sure to contact OneDigital or DBS prior to December 2026, or 90 days after termination of benefits if your medical benefits through the district were to end prior to 9/30/2026.

 

Please Note: KSD’s Moda 5 HRA is compatible with Moda’s new Garner incentive program. Please see attached or visit https://modahealth.com/oebb/garner/ for more information on Garner.

 

Please see the attached HRA Claims Guide for more information. Important: Claims for dates of service between October 1, 2025 – September 30, 2026 must be received no later than December 31, 2026, or 90 days after termination of benefits if your medical benefits through the district were to end prior to 9/30/2026.

Accessing your Group HRA Account Online

You may visit https://www.dbsbenefits.com/ to obtain access to your HRA account information online. The first time you visit, you will need to set up a user name and password. The PIN number for your employer is KnappaSD (not case sensitive).

 

NEXT STEPS

Direct Deposit

If you have not yet done so, please return a completed DBS Direct Deposit Application (these can be found on the HRA Plan Documents link on the attached HRA Resource document) using whichever of the following options works best for you:

  1. Fax directly to DBS at (262)367-5938
  2. Submit on the DBS website or mobile app as though the Direct Deposit application were an EOB that you are planning to submit

This form is used for you to designate which account you would like DBS to use to get funds to you once you are eligible for disbursements. The document asks you to attach a copy of a voided or cancelled check, but there is no need to do so as long as your handwriting (or typing) is accurate and legible. You may also type or use a digital signature.

 

If DBS has your banking information on file, HRA disbursements will show on your banking account from “HRA Claims Div Ben Services”.
